How to become a new patient at Muncie Primary Care

1. Click here for our online application form or contact our office so we may complete an application over the phone. If you don’t have a preference or know which doctor to choose, we can choose an available one for you

2. The selected provider will look over your application and gauge whether you and your current medical needs will be best served given the doctor’s expertise and available time. The providers at Internists Associated wish to do their very best work for each and every patient they take on.

3. If the chosen provider cannot meet your needs at the present time, our office staff can recommend another provider in the practice if one is available. 

4. Once your application is accepted, our office staff will schedule a first appointment to become acquainted with you and your medical needs—and hopefully start a long, healthy relationship.
